1–5 star rating
One tap answers the main question. It is the only field customers have to fill in.
A feedback QR code lets customers rate their experience from 1 to 5 stars and add a comment right where it happens – at the table, the counter or the front desk. Customers who are happy to share it publicly can continue to your Google review page.
You need a free Qdestera account, because the feedback page is hosted for you. Setting it up takes a few minutes.
Enter your business name and the main question, such as “How was your visit today?”. Add a subtitle, a short description and images if you want to set the scene.
Add optional criteria like service, cleanliness or speed that customers score separately. Decide whether to ask for an email or phone number, and write your thank-you message and a message for low ratings.
Paste your Google review link, set the rating from which the review button appears and choose whether customers are sent there automatically.
Pick one of three page templates and an accent color, style the QR code with your colors and logo, and download it for print.
A feedback QR code is a printed invitation to tell you how things went. Customers scan it with the phone camera, with no app and no account, and a short page opens: a star rating, a few optional criteria and a comment box. It takes less time than finding a pen for a paper comment card.
People often search for it as a survey QR code, and it covers the most common survey need – a quick satisfaction check. It is deliberately short: one main question, a rating and room for words. Short forms are finished more often than long questionnaires, especially on a phone.
Because it is a dynamic QR code, the page behind the printed code stays editable. Change the question for a new season, add a criterion or switch templates, and every table card and sticker already in use shows the new version.
A clean, mobile page in your colors that asks only what you chose to ask.
One tap answers the main question. It is the only field customers have to fill in.
Add topics such as service, taste or cleanliness, each with its own star score, to see where an experience shone or slipped.
Room for the details a star cannot express – a friendly waiter's name or a slow check-in.
Turn on the email or phone field when you want to follow up. Customers can also add their name, or stay anonymous.
Show a thank-you message after sending, plus a separate, understanding message for low ratings.
Customers can continue to your Google review page with one tap, or be taken there automatically.
After a customer sends their rating, Qdestera can show a button to your Google review page. You choose the minimum rating for that button, and you can switch on automatic forwarding so customers land on Google without an extra tap.
A word of caution: Google's review guidelines discourage “review gating”, which means asking only satisfied customers for public reviews. We recommend offering the review step to every customer. Set the minimum rating to one star, or place a separate Google review QR code where every guest sees it.
Low ratings still deserve care. Your low-rating message can apologize, explain what happens next and, if you ask for contact details, promise a personal reply. Customers who feel heard are more likely to give you a second chance.
Feedback is only useful if someone reads it. Every rating and comment lands on the QR code's own responses screen.
The summary shows the average rating and how many 1- to 5-star responses you received in the selected period, so a drop after a busy weekend stands out immediately.
Download the filtered responses as a CSV file that opens cleanly in Excel or Google Sheets. Names and contact details that visitors chose to leave are visible only to owners and admins, and every export that includes them is recorded in the audit log.
Ask while the experience is fresh – on site, in the moment.
Put the code on table tents or the bill folder, next to your QR code menu.
Place it in rooms and at reception so guests can speak up before checkout. See more ideas for hotels.
Print it on receipts, counter displays or the fitting room mirror.
Hand it over with the appointment card or show it in the waiting area.
Show the code on the last slide so participants rate the session before they leave.
Add a small card to the parcel and ask how the order arrived.
For long questionnaires with many question types, link a URL QR code to your survey tool. For quick, frequent satisfaction checks, the built-in page is faster.

The end of a meal, checkout or the last minutes of a training work better than the entrance.
“How was your visit today?” gets clearer answers than “Rate us”.
A short line such as “Your feedback goes straight to our team” shows it will be read – as long as that is true.
Use two to four criteria at most. Every extra field costs you finished responses.
Turn on the contact field only if you really plan to reply, and handle personal data according to your privacy obligations.
Do not offer discounts or gifts in exchange for Google reviews; Google's rules do not allow incentivized reviews.
Replace them with a feedback QR code that customers actually fill in, and give happy guests an easy way to review you on Google.

The feedback page asks one main question with a star rating, plus optional criteria that each get their own score, a comment box and contact fields. For long surveys, use a URL QR code that opens your survey tool.
